Título : Factores que intervienen en la adherencia al tratamiento nutricio y farmacológico, en pacientes con enfermedad renal crónica en hemodiálisis, en el municipio de Querétaro

Descripción : Tanto el tratamiento nutricio como farmacológico son pilares para el tratamiento del paciente con ERC en hemodiálisis; ya que se encuentran sujetos a diversas situaciones psicosociales que interfieren con la adherencia, poco se ha estudiado acerca de los aspectos que intervienen en la adherencia a los tratamientos. El objetivo de este estudio fue identificar los factores que intervienen en la adherencia al tratamiento nutricio y farmacológico, de pacientes con enfermedad renal crónica en hemodiálisis, en el municipio de Querétaro. Fue un estudio transversal descriptivo. Para medir la adherencia a dieta y al tratamiento farmacológico se diseñaron dos cuestionarios, ambos fueron validados por criterio de contenido de expertos, una vez validados se aplicaron, así como también un estudio socioeconómico y tres recordatorios de 24 horas; también se utilizaron parámetros de buen control: potasio y fósforo séricos además de ganacia de peso interdialítica. Para el análisis estadístico se utilizaron porcentajes y frecuencias, ANOVA y regresión logística. Participaron 40 pacientes con ERC con hemodiálisis, 35% mujeres y 65% hombres. El 30% presentaron buena adherencia, el 70% restante presentaron regular y no adherencia a dieta; de los factores estudiados, se observó que la presencia de comorbilidad, menor tiempo con HD (≤1 año) y de diagnóstico de ERC y acudir 1 vez cada 2 meses a consulta de nutrición, disminuye el riesgo de no adherirse a la dieta. Los participantes manifestaron que los factores que dificultan mayormente la adherencia a la dieta son: las restricciones de líquido, sodio y consumo de leguminosas. En cuanto a la adherencia al tratamiento farmacológico el 57.5% de los pacientes presentaron una buena adherencia, y el 42.5% regular y no adherencia, de las variables estudiadas se observó que acudir con frecuencia a consulta médica (1 vez por mes) y presentar comorbilidad, disminuye el riesgo de no adherirse. Los participantes manifestaron que los principales factores que dificultan la adherencia son: la disponibilidad del medicamento y el olvido en las tomas. Se puede concluir que existe falta de adherencia a la dieta y en menor medida falta de adherencia al tratamiento farmacológico. También se puede observar que la adherencia puede ser mediada por diversos factores sociodemográficos, clínicos y sociales; la comprensión de estos factores permitirá un abordaje más integral para el paciente e incluso podría favorecer la adhesión al tratamiento.

The objective of this study is to identify the factors that intervene in the adherence to nutritional and pharmacological treatment of patients with chronic kidney disease in hemodialysis, in the municipality of Querétaro. It was a descriptive cross-sectional study. To measure adherence to diet and pharmacological treatment, two questionnaires were designed, both were validated by expert content criteria, once they were validated they were applied, as well as a socioeconomic study and three 24-hour reminders; Good control parameters were also used: serum potassium and phosphorus as well as interdialytic weight gain. Percentages and frequencies, ANOVA and logistic regression were used for the statistical analysis. Forty patients with CKD with hemodialysis, 35% women and 65% men participated. 30% presented good adherence, the remaining 70% presented regular and no adherence to diet; of the factors studied, it was observed that the presence of comorbidity, less time with HD (≤1 year) and diagnosis of CKD and go 1 time every 2 months to nutrition consultation, reduces the risk of not adhering to the diet. The participants stated that the factors that make adherence to the diet more difficult are: the restrictions of liquid, sodium and consumption of legumes. Regarding the adherence to the pharmacological treatment, 57.5% of the patients showed a good adherence, and 42.5% regular and non-adherent, of the studied variables, it was observed that they frequently go to the doctor's office (1 time per month) and present comorbidity, decreases the risk of not adhering. The participants stated that the main factors that hinder adherence are: the availability of medication and forgetfulness in the takings. It can be concluded that there is a lack of adherence to the diet and to a lesser extent lack of adherence to pharmacological treatment. It can also be observed that adherence can be mediated by various sociodemographic, clinical and social factors; Understanding these factors will allow a more comprehensive approach for the patient and may even favor adherence to treatment.
