Título : Estandarización de la técnica de técnica quirúrgica de Interposición Ileal en ratas Wistar macho.

Descripción : En pacientes sometidos a cirugía bariátrica se ha observado un aumento significativo en los niveles circulantes de la proteína Glucagon-like peptide 1 (GLP-1) en el posquirúrgico inmediato en cirugías que anteriorizan al íleon, incluso antes de la pérdida de peso. Se ha demostrado que la cirugía de Interposición Ileal, que forma parte de las técnicas de la cirugía bariátrica y consiste en la transposición del segmento anterior del íleon a nivel del ligamento de Treitz, promueve el contacto temprano del bolo alimenticio con el íleon e incrementa la secreción de GLP-1. Esta proteína es sintetizada por las células L intestinales y por las neuronas preproglucagon en el cerebro, uniéndose a un receptor específico con localización ubicua, provocando múltiples efectos. Entre otras cosas, GLP-1 afecta la regulación la glucosa sanguínea, sin embargo, es rápidamente degradada por la dipeptidil péptidas 4 (DPP-4), lo cual dificulta su cuantificación. Objetivo: Estandarizar la técnica quirúrgica de Interposición Ileal en un modelo murino para posteriormente, determinar si se logra un incremento de GLP-1 respecto al nivel basal previo. Metodología: Fueron utilizadas ratas Wistar, con un peso >500 g, sometidas a interposición Ileal mediante dos técnicas distintas: a) anastomosis íleon-yeyuno entero-entero latero-lateral en punto plano continuo y b) transposición ileal. Resultados: Con la técnica original de Interposición Ileal, las ratas sobrevivieron menos de 18 horas, lo cual impidió evaluar la funcionalidad del intestino. Por otro lado, la técnica modificada íleon-yeyuno entero-entero anastomosis latero-lateral en punto plano continuo, permitió una sobrevida del 20% mayor a 4 semanas. Conclusión: La técnica de anastomosis íleon-yeyuno entero-entero latero-lateral permitió obtener una mejor sobrevida respecto de la técnica original de Interposición Ileal por la disminución del número de anastomosis, lo cual permitió una mínima invasión, reduciendo el sangrado y disminuyendo la hipoxia intestinal en la anastomosis.
In patients undergoing bariatric surgery, a significant increase in circulating levels of the Glucagon-like peptide 1 (GLP-1) protein has been observed in the immediate postoperative period in surgeries that precede the ileum, even before weight loss. It has been demonstrated that the Ileal Interposition Surgery, which is part of the bariatric surgery techniques and consists in the transposition of the anterior segment of the ileum at the level of the Treitz ligament, promotes the early contact of the alimentary bolus with the ileum and increases the GLP-1 secretion. This protein is synthesized by the intestinal L cells and by the neurons preproglucagon in the brain, joining a specific receptor with ubiquitous localization, causing multiple effects. Among other things, GLP-1 affects the regulation of blood glucose, however, it is rapidly degraded by dipeptidyl peptides 4 (DPP-4), which makes its quantification difficult. Objective: To standardize the surgical technique of Ileal Interposition in a murine model to later determine if an increase of GLP-1 is achieved with respect to the previous basal level. Methodology: Wistar rats weighing> 500 g were used, subjected to Ileal interposition by two different techniques: a) ileo-entero antero-jejunum entero-entero latero-lateral in continuous flat point and b) ileal transposition. Results: With the original technique of Ileal Interposition, the rats survived less than 18 hours, which prevented evaluating the functionality of the intestine. On the other hand, the modified ileo-jejunum-whole-latero-lateral anastomosis technique in continuous flat point, allowed a survival of 20% greater than 4 weeks. Conclusion: The latero-lateral ileum-entero-entero anastomosis technique allowed obtaining a better survival compared to the original technique of Ileal Interposition due to the decrease in the number of anastomosis, which allowed a minimal invasion, reducing bleeding and decreasing hypoxia intestinal in the anastomosis.
